Abstract
El trauma con hemorragia masiva continúa siendo una causa de morbimortalidad importante y un reto para los profesionales en anestesiología. Clasificarlo de una forma adecuada para prever que pacientes se encuentran en riesgo de requerir transfusión masiva y resucitación enérgica es un primer paso para su abordaje efectivo. Generar un protocolo basado en recomendaciones recientes facilita la toma de decisiones y acelera procesos en un escenario donde el tiempo es apremiante. En el siguiente trabajo revisamos varios aspectos de esta patología y generamos recomendaciones generales y un protocolo inicial para el manejo de la hemorragia asociada a trauma en el Hospital México, ubicado en San José, Costa Rica, el cual es uno de los hospitales de tercer nivel parte de la Caja Costarricense del Seguro Social.
Trauma associated with massive hemorrhage continues to represent an important morbimortality cause and a challenge to anesthesiology professionals. Classifying it in an adequate manner to preemptively define which patients are at risk of requiring massive transfusion and aggressive resuscitation is the first step for its effective approach. Orienting initial therapeutic attitude with a standardized protocol based on recent recommendations facilitates decision-making and accelerates interventions on a clinical setting in which time is of the essence. On the following work several aspects of this pathology are reviewed, and general recommendations are proposed, as well as an initial protocol for management of trauma associated hemorrhage.
