Propuesta de protocolo de abordaje diagnóstico y seguimiento de sospecha y confirma de enfermedad tromboembólica pulmonar crónica

Abstract
La enfermedad pulmonar tromboembólica crónica (CTEPD) es un término que engloba a la enfermedad obstructiva crónica de las vasculatura pulmonar. Cuando se habla de este tema, se hace referencia tanto a aquella con hipertensión pulmonar (llamada hipertensión pulmonar tromboembólica crónica, por sus siglas CTEPH), como sin hipertensión arterial (esta última es una entidad patológica infrecuente, pero con alta morbimortalidad en las personas afectadas). En la última década se han realizado grandes avances en estudios tanto en diagnóstico como en tratamiento de esta enfermedad. Este documento consta de tres partes. Se buscó de manera exhaustiva la información publicada sobre la enfermedad para realizar un marco teórico adecuado según las publicaciones más recientes y relevantes para definir la población que se beneficiaría de un abordaje de seguimiento y diagnóstico. Se indagó sobre las herramientas de exclusión y diagnóstico de la enfermedad, así como las estrategias terapéuticas intervencionistas, farmacológicas y no farmacológicas con mayor evidencia para resultados positivos en los pacientes diagnosticados. Como segunda parte se realizó una propuesta de un protocolo de diagnóstico y confirmación a una población de riesgo de presentar la enfermedad, y posteriormente un protocolo de manejo con las terapias que se cuentan a nivel institucional. Por último, se identificaron las brechas en cuanto a conocimiento y prácticas a nivel institucional para poder aplicar el protocolo de forma exitosa. En el presente estudio se concluye que, si bien se cuenta con herramientas para un seguimiento y diagnóstico adecuado a población de riesgo, todavía se está con limitación para el manejo óptimo por recomendaciones internacionales por guías y por estudios pivotales en la terapéutica de estos pacientes.
Chronic thromboembolic pulmonary disease (CTEPD) is a term that encompasses chronic obstructive disease of the pulmonary vasculature, both with pulmonary hypertension, known as chronic thromboembolic pulmonary hypertension (CTEPH), and without arterial hypertension, which is an infrequent pathological entity but has high morbidity and mortality in affected individuals. Over the last decade, significant advances have been made in both the diagnosis and treatment of this disease. This document consists of three parts. An exhaustive search was conducted for published information on the disease to create an appropriate theoretical framework based on the most recent and relevant publications, aimed at defining the population that would benefit from a follow-up and diagnostic approach, as well as the exclusion and diagnostic tools for the disease, and the interventional, pharmacological, and non-pharmacological therapeutic strategies with the most evidence for positive outcomes in diagnosed patients. The second part includes a proposal for a diagnostic and confirmation protocol for a population at risk of developing the disease, followed by a management protocol with therapies available at the institutional level. Finally, an identification of the gaps in knowledge and practices at the institutional level was conducted in the hopes to successfully implement this protocol. This study concludes that while we have tools for proper follow-up and diagnosis of at-risk populations, we still face limitations mainly in optimal management due to international recommendations, guidelines, and pivotal studies in the therapy of these patients.
