Consideraciones en el uso de anestesia neuroaxial en pacientes adultos con terapia de anticoagulación

Abstract

Las técnicas neuroaxiales son algunos de los procedimientos que más caracterizan a los anestesiólogos y que se realizan de forma diaria. Su versatilidad y beneficios que hoy en día se siguen describiendo las vuelven una excelente opción como técnica anestésica y analgésica. Con el avance de la medicina y la tecnología, la prevalencia de enfermedades crónicas del sistema cardiovascular y otras enfermedades han aumentado la necesidad de utilizar fármacos anticoagulantes y antiagregantes con el objetivo de disminuir complicaciones trombóticas para mejorar la sobrevida de las personas. Para demostrar la seguridad de estas técnicas, según la evidencia actual, se realizó una revisión acerca de las ventajas del uso de las técnicas neuroaxiales, sus indicaciones y contraindicaciones. Al mismo tiempo, se abordará el concepto del hematoma espinal epidural, cómo realizar el diagnóstico y su manejo adecuado. Por otra parte, se describirá la estratificación de riesgo, esquemas de tromboprofilaxis más comunes en diferentes grupos poblacionales y la farmacodinamia de la terapia antitrombótica más utilizada. Finalmente, se estudiaron las guías y recomendaciones más actuales en cuanto al manejo perioperatorio de los fármacos antitrombóticos, la realización de técnicas regionales y posibles reversiones en estados de anticoagulación. Por otra parte, se creó una propuesta de algoritmo para el manejo en estos escenarios con el fin de ampliar las opciones terapéuticas en estos pacientes y brindar a este grupo poblacional todas las ventajas descritas y mejorar su atención durante la estancia hospitalaria.

Neuraxial techniques are some of the procedures that most characterize anesthesiologists and that are performed daily. Their versatility and benefits that are still being described today make them an excellent option as either an anesthetic or an analgesic technique. With the advancement of medicine and technology, the prevalence of chronic diseases of the cardiovascular system and other diseases have increased the need to use anticoagulant and antiplatelet drugs in order to reduce thrombotic complications to improve people's survival. To demonstrate the safety of these techniques, based on current evidence, a review was carried out on the advantages of using neuraxial techniques, their indications, and contraindications. At the same time, the concept of spinal epidural hematoma, how to make the diagnosis and its proper management will be addressed. On the other hand, risk stratification, the most common thromboprophylaxis regimens in different population groups, and the pharmacodynamics of the most widely used antithrombotic therapy will be described. Finally, the most current guidelines and recommendations regarding the perioperative management of antithrombotic drugs, the performance of regional techniques and possible reversals in anticoagulation states were studied. On the other hand, an algorithm proposal was created for the management in these scenarios to expand the therapeutic options in these patients and provide this population group with all the advantages described and improve their care during the hospital stay.
