Abstract
La cirugía de amputación de miembro inferior es un procedimiento de alto riesgo cardiovascular, especialmente en pacientes con insuficiencia cardiaca con fracción de eyección reducida (ICFEr). En esta población, la estabilidad hemodinámica es esencial para reducir en mayor medida la morbilidad y mortalidad. Entre las técnicas anestésicas disponibles, la anestesia regional, en comparación con la anestesia general o neuroaxial, se destaca por su bajo impacto hemodinámico y sus beneficios, como la reducción de complicaciones perioperatorias, menor necesidad de cuidados intensivos, transfusiones, y analgesia de rescate. Este trabajo se enfoca en destacar la importancia de elegir adecuadamente la técnica anestésica a la hora de abordar los pacientes con ICFEr sometidos a una amputación de transtibial y transfemoral, exponer los beneficios de la anestesia regional y describir los bloqueos de los nervios implicados en estos procedimientos
Lower limb amputation surgery is a procedure with a high cardiovascular risk, especially in patients with heart failure with reduced ejection fraction (HFrEF). Maintaining hemodynamic stability in this population is essential to significantly reduce the morbidity and mortality. Among the anesthetic techniques available, regional anesthesia, compared to general or neuraxial anesthesia, stands out for its low hemodynamic impact and its benefits, such as reducing perioperative complications, the need for an intensive care unit, transfusions, and rescue analgesia. This work focuses on emphasizing the importance of selecting the appropriate anesthetic technique when addressing patients with HFrEF undergoing transtibial and transfemoral amputation, expose the benefits of regional anesthesia and describe the nerve blocks involved in these procedures
