Reconstrucción del proceso del programa de intervención temprana realizado a pacientes de la Clínica de Primeros Episodios Psicóticos del Hospital Nacional Psiquiátrico como acercamiento a la implementación del acompañamiento terapéutico como dispositivo

Abstract

La presente investigación se interesó por estudiar el camino trazado para la atención de las personas que se presentan a la Clínica de Primeros Episodios Psicóticos del Hospital Nacional Psiquiátrico y analizar dicho proceso con los matices teóricos del Acompañamiento Terapéutico, de tal forma que se pueda realizar un acercamiento a la posibilidad de enmarcar este último dentro de la atención del sistema hospitalario.

El Acompañamiento Terapéutico como dispositivo de abordaje al tratamiento de la psicosis, es un campo de reciente estudio, se señala que a partir de los años sesenta inicia la historia del dispositivo como un tratamiento de abordaje múltiple, por lo tanto, viene a ser una forma de entender y hacer con el sufrimiento subjetivo, más allá de un modelo biomédico y positivista, se dirige a dar una mirada distinta al padecer del otro, otorgándole un lugar como sujeto deseante y un actor social constructor de su realidad.

Se propuso como objetivo central, reconstruir el proceso del programa de intervención temprana realizado a pacientes de la Clínica de Primeros Episodios Psicóticos como acercamiento a la implementación del Acompañamiento Terapéutico como Dispositivo.

La estrategia metodológica diseñada contempló tres fases, la Fase I incluyó el proceso de revisión de antecedentes, problematización del tema y definición de objetivos. En la Fase II se efectuó una revisión documental, entrevistas a informantes clave y la reconstrucción de estudios de caso en total participaron quince personas y la Fase III correspondió a la sistematización y análisis de la información.

Dentro de las principales conclusiones se señala que la sistematización del proceso de Intervención de la Clínica PEP integra la subsecuente serie de pasos que permiten el ingreso, atención y seguimiento de las personas que han presentado un primer episodio psicótico en el cual se detallan:

Referencias, valoración por parte de Psiquiatría, definición previa de objetivos por parte de la coordinación del equipo hacia los demás profesionales, para la atención y delimitación de objetivos a nivel individual por parte de cada disciplina y seguimiento.

De acuerdo con los hallazgos encontrados en los estudios de casos, la intervención interdisciplinaria es fundamental en las primeras fases de recuperación de las personas que han presentado un Primer Episodio Psicótico, porque permite ampliar las posibilidades de re-inserción psicosocial de acuerdo con las particularidades y necesidades que presenta cada persona y su familia, además, esto permite ir más allá del tratamiento farmacológico.

Por lo tanto, es posible la inscripción del Acompañamiento como Dispositivo dentro de la Clínica de Primeros Episodios Psicóticos, como recurso alterno a los tratamientos tradicionales, los diferentes actores clave de este proceso de investigación han observado la importancia de insertar alternativas que posibiliten mejorar el alcance de lo que hasta el día de hoy se realiza, variables que conjugadas permiten entrever esta posibilidad como real.

No obstante, se deben superar ciertos retos para la puesta en práctica del acompañamiento terapéutico, por ejemplo: modificar la normativa interna de tal manera que se legitime el dispositivo de acompañamiento terapéutico como recurso dentro de la práctica en salud mental a nivel institucional y se realicen los ajustes necesarios para su funcionamiento y fortalecer el recurso humano para poder incluir este dispositivo como recurso de atención para la población.

This research was interested in studying the path outlined for the care of people who attend to the Clinic for First Psychotic Episodes of the National Psychiatric Hospital and analyze this process with the theoretical nuances of Therapeutic Accompaniment, in such a way that an approach can be done to the possibility of framing the latter within the care of the hospital system. Therapeutic Accompaniment as an approach device to the treatment of psychosis is a field of recent study, it is pointed out that from the 60s, the history of the device began as a multiple approach treatment, therefore, it became a form to understand and do with subjective suffering, beyond a biomedical and positivist model, it is directed to give a different look to the suffering of the other, granting them a place as a desired subject and a social actor constructor of their reality. It was proposed as a central objective, to reconstruct the process of the early intervention program carried out with patients of the Clinic for First Psychotic Episodes as an approach to the implementation of Therapeutic Accompaniment as a device. The designed methodological strategy included three phases, Phase I included the process of background review, problematization of the issue and definition of objectives. In Phase II, a documentary review was carried out, interviews with key informants and the reconstruction of case studies in total, fifteen people participated, and Phase III corresponded to the systematization and analysis of the information. Among the main conclusions, it is pointed out that the systematization of the PEP Clinic Intervention process of PEP Clinic integrates the subsequent series of steps that allow the entry, attention and, follow-up of the people who have presented a first psychotic episode in which it is detailed: references, evaluation by Psychiatry, previous definition from objectives by the coordination of the equipment for the other professionals, to the attention and definition of objectives at the individual level by each discipline and follow-up. Based on the characteristics found in the case studies, the interdisciplinary intervention is fundamental in the first stages of recovery of the persons that a First Psychotic Episode has presented, because it allows to expand the possibilities of psychosocial re-insertion of acknowledgment with the particularities and the needs that each person and their family have. Moreover, this allows you to go further in pharmacological treatment. From the completion of the study, it was concluded that it is possible to enroll the accompaniment as a device within the Clinic for First Psychotic Episodes, as an alternative resource to traditional treatments, the different key actors in this research process have observed the importance of inserting alternatives that make it possible to improve the scope of what is being done to this day, variables that together allow us to glimpse this possibility as real. However, certain challenges must be overcome for the implementation of therapeutic accompaniment, for example: modifying the internal regulations in such a way that the therapeutic accompaniment device is legitimized as a resource within the practice in mental health at the institutional level and they are carried out the necessary adjustments for its operation and to strengthen the human resource to be able to include this device as a care resource for the population.
