dc.description.abstractEl presente estudio pretende analizar la base de datos del banco de ojos de la CCSS, con la intensión de estudiar la demografía de la patología corneal asociada a trasplante de cornea en el país y las tendencias en cirugía de trasplante de cornea durante las ultimas dos décadas. Además de identificar las diferencias observables en los periodos previos y posterior a la entrada en vigencia de la ley 9222 en cuanto a la capacidad institucional de captación de tejidos y por lo tanto de su utilización para tratar pacientes con patología corneal, la calidad de tejido corneal captado (según determinantes indirectos como la edad del donador) y el tiempo en lista de espera para el paciente que requiere de un procedimiento de trasplante. El diseño metodológico consiste en un estudio retrospectivo de revisión de base de datos en su mayoría. Dado que la base de datos ha sufrido modificaciones para ajustarse a los requerimientos de las autoridades en salud y la realidad nacional, se utilizo además un muestreo irrestricto aleatorio con 95% de confiabilidad, permitiendo una probabilidad de error máximo del 5% para la recolección de datos de las variables que se desea estudiar y que no constaran en la base de datos para todos los periodos estudiados, Para el análisis de las variables se utilizó estadística descriptiva, comparaciones entre medidas de tendencia central, y un modelo de regresión logística multivariado. Los criterios de inclusión consisten a la totalidad de los datos contenidos en la base datos del banco de ojos de la CCSS, sin que ningún dato se excluyera según edad, genero, etnia ni origen geográfico. Se excluyó del análisis las líneas de registro incompletas, esto es, cuando para un paciente registrado en la base de datos, se omitió incluir variables que si correspondía tabular para el año en cuestión. Las variables estudiadas Fueron: Cantidad de Trasplantes realizados por año, Cantidad de tejidos captados, Tiempo de espera por trasplante, Edad del receptor, Edad al fallecimiento, Sexo del donante fallecido, Provincia de residencia del receptor, Centro donde se realiza el trasplante, Establecimiento de origen del receptor, Tipo de muerte, Causa primaria de muerte del donador, Fecha solicitud de trasplante, Fecha de trasplante, Diagnóstico del paciente receptor, Tejido rechazado(Si/no), Causa de rechazo, Lateralidad del ojo trasplantado y Tipo de cirugía. Conclusiones: El Presente estudio logra sentar precedente al describir las tendencias la patología corneal responsable de trasplante de cornea en nuestro país durante las ultimas dos décadas, las tendencias en captación de tejidos, y las variables relacionadas tanto a los pacientes trasplantados como a los pacientes donadores de tejido corneal en nuestro país. Además, se logró evidenciar diferencias importantes al comparar los periodos antes y después del año 2014. Es importante mencionar que, aunque este estudio si logra evidenciar tendencias en los análisis longitudinales, y diferencias al comparar los dos periodos estudiados, es evidente que existen múltiples factores que influencian a las variables y que es muy probable que las modificaciones en el marco legal como la entrada en vigencia de la ley 9222 y su reglamento expliquen únicamente parte de los cambios observados. Este trabajo sienta una base para futuros trabajos clínicos, y para comparaciones respecto a la realidad nacional y la epidemiología de la patología relacionada con trasplante de cornea en nuestro país.  es_ES
dc.description.abstractThe present study aims to analyze the database of the CCSS national Eye Bank, with the intention of studying the demography of corneal pathology associated with corneal transplantation and trends in corneal transplant surgery in our country during the last two decades. In addition to identifying the observable differences in the periods before and after the year 2014, when the law 9222 came into force, in terms of the institutional capacity for tissue uptake and therefore its use to treat patients with corneal pathology, the quality of corneal tissue captured (according to indirect determinants such as the age of the donor) and the time on the waiting list for the patient who requires a transplant procedure. The methodological design consists mostly of a retrospective study of database review. Given that the database has undergone modifications to adjust to the requirements of the health authorities and the national reality, an unrestricted random sampling with 95% reliability was also used, allowing a maximum error probability of 5% for the collection of data of the target variables but that were not included in the database for all the periods studied. For the analysis of the variables, descriptive statistics, comparisons between measures of central tendency, and a multivariate logistic regression model were used. The inclusion criteria consist of all the data contained in the CCSS eye bank database, without any data being excluded according to age, gender, ethnicity or geographical origin. Incomplete record lines were excluded from the analysis, that is, when information was registered in the database while omitting to include variables that should have been tabulated for the year in question. The variables studied were: Number of transplants performed per year, Number of tissues captured, Waiting time for transplantation, Age of the recipient, Age at death, Sex of the deceased donor, Province of residence of the recipient, Center where the transplant is performed, Establishment of origin of the recipient, Type of death, Primary cause of death of the donor, Date of transplant request, Date of transplant, Diagnosis of the recipient patient, Rejected tissue (Yes / no), Rejection cause, Laterality of the transplanted eye and Type of surgery . Conclusions: The present study manages to set a precedent by describing the trends in the corneal pathology responsible for corneal transplantation in our country during the last two decades, the trends in tissue uptake, and the variables related to both transplant patients and donor patients. of corneal tissue in our country. In addition, it was possible to show important differences when comparing the periods before and after 2014. It is important to mention that although this study does show trends in the longitudinal analyzes, and differences when comparing the two periods studied, it is evident that there are multiple factors that influence the variables and that it is very likely that modifications in the legal framework such as the entry into force of Law 9222 and its regulations explain only part of the observed changes. This work lays a basis for future clinical studies, and for comparisons with respect to the national reality and the epidemiology of the pathology related to corneal transplantation in our country.es_ES
