Situación Actual de Egresos Hospitalarios asociados a Enfermedad de Wilson en Costa Rica

La Enfermedad de Wilson (EW) tiene alta incidencia en Costa Rica y existe poca
información epidemiológica publicada al respecto en el país. Este trabajo tiene como objetivo
actualizar la información local de esta enfermedad con respecto a su comportamiento en
hospitales. Para esto, se analizaron los egresos hospitalarios debidos a Enfermedad de Wilson
desde 1997 y hasta el año 2017, según los datos públicos, proporcionados por la Caja
Costarricense del Seguro Social por medio del Área de Estadísticas en Salud. Los datos
fueron analizados con respecto al sexo, edad, año y centro de salud en el cual se da el egreso
hospitalario.
Entre los principales hallazgos destaca que los años con mayor cantidad de egresos
hospitalarios fueron el 2004, 2011, 2012, 2013 y el 2014, siendo particularmente alta la
cantidad de egresos en el año 2011: 77 egresos. Además, en 11 de los 20 años analizados, el
egreso hospitalario fue mayor en hombres. De forma general la mayor cantidad de egresos se
ha dado en pacientes entre los 20 y 44 años. Históricamente la mayor cantidad de egresos
hospitalarios se ha dado en los hospitales San Juan de Dios, Hospital México y el Hospital
Nacional de Niños. En conclusión, la enfermedad de Wilson es una enfermedad relativamente
común en Costa Rica, su registro hospitalario se concentra en el Gran Área Metropolitana y
teniendo en cuenta la mortalidad de sus complicaciones es necesario mantener una vigilancia
epidemiológica estricta para su diagnóstico, tratamiento y seguimiento. Wilson's Disease (WD) is of high incidence in Costa Rica, and there is insufficient
epidemiological information related published in the country. The aim of our work is to
update the local behavior of hospitalizations related with the disease. Hospital outpatient data
for WD from the years of 1997 to 2017 was analyzed according to the public information
provided by the Costa Rican Department of Social Security through the Health Statistics Area
(Area de Estadísticas en Salud de la C.C.S.S). The data were analyzed by sex, age, year, and
health center in which the hospital discharge was given.

It was detected that the five years with the highest number of hospital discharges were 2004,
2011, 2012, 2013 and 2014. In 2011 a record number of 77 releases due to WD was reported.
The year with the lowest reported was 1997. In addition, in 11 of the 20 years reported, the
number of women for whom hospital discharge was reported was higher than the number of
men. Overall, the most considerable amount of discharges occurred for patients between 20-
44 years of age. Historically the most significant amount of hospital discharges has occurred
in Hospital San Juan de Dios, Hospital México, and Hospital Nacional de Niños. Wilson's
disease is a relatively common disease in Costa Rica, and its hospital registry is concentrated
in the Greater Metropolitan Area. Considering the mortality of its complications it is
necessary to maintain strict epidemiological surveillance for its diagnosis, treatment and
clinical follow-up.
