Creatina intraeritrocítica como valoración de la edad celular

El índice de reticulocitos es el parámetro más utilizado para medir la producción eritrocitaria medular. Sin embargo, como éste ha sido asociado con cierto grado de error estadístico, se ha buscado un método objetivo y de fácil realización, la determinación de creatina intraeritrocítica, para obtener una mayor información acerca de la edad eritrocitaria en determinados procesos hemolíticos. La misma se determinó por el método del diacetil alfa-naftol, observándose una linealidad hasta 400 mg/l. Los valores de creatina obtenidos en el grupo control (n=100) fueron de 24 a 69 mg/l, y se utilizó el logaritmo de estos valores para normalizar la distribución de los datos. Se analizó un grupo de muestras de 216 pacientes con diferentes diagnósticos, obteniéndose valores aumentados del conteo de reticulocitos en un 75 por ciento de los casos de creatina intraeritrocítica en un 63 por ciento. Sin embargo, en un 13 por ciento de las muestras se obtuvieron valores aumentados de creatina intraeritrocítica con un conteo de reticulocitos normal o disminuido. Sin restarle su debida importancia a la determinación de reticulocitos, se recomienda realizar el análisis de creatina como dato complementario en el estudio de aquellos síndromes hemolíticos idiopáticos que cursen con conteos normales o subnormales de reticulocitos. The reticulocyte count is the most used parameter to measure the medular erythrocyte production. However, since it has been associated with certain degree of statiscal error, the investigators have been looking for a new and objetive method, like the determination of erythrocyte creatine, to obtain more information concerning the erythrocyte age in age in certain hemolytic anemias. Creatine was determined by the diacetyl-alfa-naphthol reaction. The method was lineal up to 400 mg/l. The creatine values obtained from the control group (n=100) were from 24 to 69 mg/I and the distribution of the data was normalized with the logarithm of the values of CRI A group of 216 samples from patients with different diagnoses were analyzed, obtaining high values of the reticulocyte count and creatine determination in 75% and 63% of the cases respectively. However, 13% of the samples had elevated values of creatine with a low or normal count of reticulocytes. Without underestimating the importance of the retyculocite count, it is recommended to analyse creatine levels as complementary information in the study of those idiopatic hemolytie syndroms that have with normal or subnormal reticulocyte counts.
