La hernia hiatal implica una entrada abdominal anormal en la cavidad torácica. Es clasificado
según la orientación entre la unión esofágica y el diafragma y se clasifica en tipos de I a IV. La
edad y el aumento del índice de masa corporal son factores de riesgo clave y además de las
enfermedades del metabolismo del colágeno. Las manifestaciones usuales incluyen reflujo
gástrico, náuseas, distensión abdominal, molestias torácicas y epigástricas, entre otros. El
tratamiento quirúrgico está bien estandarizado. Sin embargo, la recurrencia se observa en 15% –
60% de casos, y es difícil de manejar. Los síntomas de hernia hiatal recurrente varían ampliamente
e incluyen persistencia reflujo, disfagia y molestias permanentes, lo que lleva a un cambio marcado
en la calidad de vida. Debe existir una evaluación morfológica y funcional para determinar si los
síntomas se deben a recurrencia y entender la causa del fracaso. Rehacer la cirugía es técnicamente
difícil y desafiante, y debería solo debe usarse en pacientes sintomáticos cuyos síntomas son
definitivamente los de hernia hiatal recurrente. Hiatal hernia involves abnormal abdominal entry into the thoracic cavity. It is classified according
to the orientation between the esophageal junction and the diaphragm and is classified into types I
to IV. Age and increased body mass index are key risk factors and in addition to diseases of
collagen metabolism. The usual manifestations include gastric reflux, nausea, bloating, chest and
epigastric discomfort, among others. Surgical treatment is well standardized. However, recurrence
is seen in 15% -60% of cases, and is difficult to manage. Recurrent hiatal hernia symptoms vary
widely and include persistent reflux, dysphagia, and permanent discomfort, leading to a marked
change in quality of life. There should be a morphological and functional evaluation to determine
if the symptoms are due to recurrence and to understand the cause of failure. Redoing the surgery
is technically difficult and challenging, and should only be used in symptomatic patients whose
symptoms are definitely those of recurrent hiatal hernia.
